Appointments

At Idaho Hand Institute, our goal is to provide appropriate and efficient care to our patients.

For your upcoming appointment please:
New Patients
  • - Bring your insurance card, or insurance information.
  • - Most insurance companies require us to collect a copay, which is due at the time of service. If you do not have a copay, then $30 will be due at the time of service.
  • - If you do not have insurance, payment in full is required at the time of service, unless you set up a payment plan with our financial counselor.
  • - Complete your health profile through our PATIENT PORTAL to prevent delays/rescheduling. Contact us at 235-4263 if a link was not sent to your email or if you experience any difficulty.
  • - Bring a list of your current medications. Include dosage, frequency, and schedule (your pharmacy can print a list at your request. If you would like them to fax your list to us, our fax number is 233-4268).
  • - If you have been seen elsewhere for the same problem, please bring any related medical records, x-­ray films or disks, MRI reports, EMG reports, etc. from prior doctor visits with you when you come for your appointment.
  • - If you are insured by Medicaid, Healthy Connections requires a referral from your primary care provider.
  • - If you are insured through Indian Health Services, you must bring a signed referral from contract health.
  • - If you participate in a Connected Care health plan, a referral from your Primary Care Provider is required.
Returning Patients
  • - If your insurance has changed since your last visit, please bring your new insurance card, or insurance information.
  • - Most insurance companies require us to collect a copay, which is due at the time of service. If you do not have a copay, then $30 will be due at the time of service.
  • - If you do not have insurance, payment in full is required at the time of service, unless you set up a payment plan with our financial counselor.
  • - Let us know of any name, address, or phone changes.
  • - Let us know if any of your medications have changed and provide updates with your medical health status.
  • - If you have been seen elsewhere for the same problem, please bring any related medical records, x-­ray films or disks, MRI reports, EMG reports, etc. from prior doctor visits with you when you come for your appointment.
